Collection of Health Information -We collect personal information from you, including your name, contact details, address, medical history (including conditions, medications, and allergies), and billing information. This information enables us to provide health services. In addition to our staff, we may share relevant information with other health providers—such as your GP or specialist—to ensure continuity of care. Providing some information is optional, but if you choose not to share key medical details, we may be unable to provide appropriate care. ou may request a copy of any personal information we hold or ask for corrections if you believe it is inaccurate. Clinical Governance Bevan Michie – Registered Nurse and Health Consultant engages registered health practitioners to provide all clinical services. Under the Health Practitioners Competence Assurance Act (2003), all practitioners must hold a current Annual Practising Certificate and work within their scope of practice. Regulatory authorities have powers to investigate concerns and take disciplinary action. This ensures that all clinicians providing care on our behalf are competent and fit to practise.
Complaint Process We take all complaints seriously and are committed to a fair, simple, and timely resolution process in accordance with the Code of Health and Disability Services Consumers’ Rights. Complaints help us learn and improve our services. To make a complaint: You may complain in person, by phone, email, or through our website. Your complaint will be recorded and forwarded to the appropriate staff member. You will receive written acknowledgement within five working days unless resolved earlier. An outcome or update will be provided within 20 working days. If more time is required, we will notify you of revised timeframes and reasons. Closing a complaint: Once an outcome is communicated, the complaint will be closed. If you are dissatisfied, a re-investigation will be completed by someone not involved in the original review. Clinical Emergency Preparedness Providing clinical care in the home carries some risk. We ensure that all clinicians are trained and prepared for emergencies. All clinicians: Hold at least Level 5 NZ Resuscitation Council (or equivalent) certification. Have emergency airway equipment, artificial respiration devices, and access to a AED and Have immediate access to the 111 system for emergency ambulance requests.
